Your Smartwatch Might Know You’re Sick Before You Do — But Don’t Ditch Your Doctor Yet

Smartwatches have come a long way from step counting. They track your sleep, skin temperature, respiratory rate, blood oxygen, heart rate variability — some even flag possible sleep apnea. But can they actually tell you when you’re getting sick?

Sort of. Here’s what works and what doesn’t.

The thing wearables are genuinely good at: catching outliers. When your body breaks from its normal patterns, a smartwatch notices. That can be useful.

AFib detection is the star here. One Apple Watch study found irregular pulse alerts were confirmed as atrial fibrillation 84% of the time. That’s clinically useful because AFib has a clear physiological signal that consumer hardware can pick up reliably.

Basic sleep patterns and step counts also make the cut. Everything else? Gets murky.

Blood pressure alerts, calorie estimates, and detailed sleep-stage tracking aren’t considered reliable enough for doctors to act on. VO2 max and heart rate variability offer rough guesses at best. Daily wellness scores from Oura and Whoop run on proprietary algorithms that leave clinicians shrugging.

Even the good metrics can trigger false alarms. A spike in resting heart rate could mean your body’s fighting an infection — or it could just mean you drank too much and slept badly.

The real promise is in pattern recognition. Before you notice flu or COVID symptoms, your body starts shifting. Skin temperature, heart rate, breathing patterns — individually they don’t say much. Together, compared against your baseline, they might hint you’re coming down with something.

A Texas A&M and Stanford study found smartwatches could detect early signs of COVID-19 and influenza within hours of infection. The researchers estimated that earlier isolation and testing could cut pandemic transmission by up to 50%. That’s a big if, but it’s not nothing.

AI is creeping in too. Google’s Health Coach uses Gemini. Oura has Symptom Radar. Apple has Vitals. These tools stitch together sensor data and compare it to your baseline. The analysis mostly happens behind the scenes, though, offering little doctors can actually act on.

Best case: your watch nudges you to see a doctor sooner. Worst case: people take AI-generated health advice as gospel and skip real checkups entirely. The warnings say “consult a medical professional.” Not everyone reads those.

Bottom line? Wearables are good at spotting when something’s off. They’re not good at telling you what or why. That’s still a doctor’s job.
